多米尼克学员在中山一院接受培训
9月4日,中山大学附属第一医院迎来了四名特别的学员,他们从多米尼克中多友谊医院出发,跨越半个地球,踏上了来往广州的求学之路,正式开启为期八周的心血管影像学培训课程。
在此之前,中多友谊医院已于今年7月成立了配备心脏超声和心脏核磁共振影像设备的心血管影像学和远程医学中心。该中心由中山一院负责建设。

中山大学常务副校长、中山一院院长肖海鹏教授表示,此次心血管影像学培训课程旨在提升中多友谊医院的医疗服务水平,培养一支能够更好地服务多米尼克人民健康福祉的医师队伍。自2009年以来,中国向多米尼克派出了多支医疗队,其中有2名成员来自中山一院。

中多友谊医院是多米尼克最大的公立医院,来自急诊科的医生德维-希林福德(Devi Shillingford)表示,她希望通过此次学习提升自己的影像技能,并且更加深入了解不同医院的运营方式。心内科医生瑞秋-安德鲁(Rachel Andrew)表示,此次培训将大大提高多米尼克医生的诊断能力,他们将有机会学习许多以前未使用过的技术,例如心脏MRI、经食管超声心动图和斑点追踪超声心动图等,这些技术对他们而言是非常前沿的。“例如心脏MRI技术之前在多米尼克无法开展,以前许多当地居民不得不前往海外进行这项检查。”据悉,参加培训的另外两名学员分别来自内科和放射科。
▲超声医学科为不同医学背景的学员进行个体化教学,对已有超声基础的心内科学员进行经食道超声心动图、斑点追踪以及心脏造影等新技术的培训,对没有超声基础的内科和急诊科学员进行常规心脏超声检查的培训,同时还为学员设置腹部急症及掌上超声的培训。

此次培训结束后,中多友谊医院还将继续派出四名学员前来参加培训。肖海鹏院长表示,培训结束后,双方将继续通过远程医学中心进行医疗交流,包括讨论复杂病例等。
援多医疗队成员吴德熙副主任医师在多米尼克工作了三年。他曾两次主动延长了在多米尼克的支援时间,问及原因,他说:“我觉得很多事情还可以更上一个台阶,所以我选择留下来继续为当地医疗水平提升贡献力量。”
2021年,吴德熙副主任医师帮助中多友谊医院建立了多米尼克首个心内科,该科室配备了Holter监测仪、动态血压监护仪和临时起搏器等设备。在科室成立后不久,他为一名73岁的女性患者实施了多米尼克首例临时心脏起搏器植入手术。

今年7月,吴德熙副主任医师、中山一院放射诊断科医生彭洋与多米尼克当地同事开展了多米尼克首例心脏增强核磁共振检查。吴德熙副主任医师还为多米尼克制定了第一份针对急性ST段抬高型心肌梗死的溶栓方案。在这个人口数仅7万多的国家,他共治疗了约5000名多米尼克患者。吴德熙副主任医师表示,通过改善诊断手段和治疗方式,可以减少患者看病的费用,患者无需再前往海外就医。

此外,中国-多米尼克心血管影像学中心和远程医学中心启动时,超声医学科何薇医师曾前往多米尼克进行心脏超声带教工作,帮助当地医院规范心脏超声检查及诊断流程,开展多项“多米尼克首例”新技术,包括三维经食道超声心动图、斑点追踪技术、心脏声学造影 、肱动脉内皮功能测定、使用掌上超声在急诊床旁为患者进行超声心动图检查等。

Dominican doctors training

Four doctors from Dominica have started an eight-week training course on cardiovascular imaging at the First Affiliated Hospital of Sun Yat-sen University in Guangzhou, Guangdong province.
The training, which started on Sept 4, follows the establishment of a cardiovascular imaging and telemedicine center at the Dominica-China Friendship Hospital in the Caribbean country in July.
The First Affiliated Hospital SYSU implemented the building of the center, which is equipped with cardiac ultrasound imaging and cardiac magnetic resonance imaging equipment.
"The project aims to promote the advancement of healthcare delivery at the Dominica-China Friendship Hospital, and build a team of competent physicians who are able to better serve the health and well-being of people in Dominica," said Xiao Haipeng, president of the First Affiliated Hospital SYSU.
China has sent medical teams to Dominica since 2009, with many members coming from the First Affiliated Hospital SYSU, he said.
Devi Shillingford from the emergency department of the Dominica-China Friendship Hospital, which is the largest public hospital in the country, said she expected to improve her imaging skills and to see how a different hospital operates.
Rachel Andrew from the cardiology department said: "It actually is going to be a tremendous improvement in our diagnostic capabilities. Many of the modalities that we are going to practice were not offered before."
Things like contrast cardiac MRI, transesophageal echocardiography and speckle tracking echocardiography are new to them.
"It's going to improve our ability to make a correct diagnosis, including using the cardiac MRI.Many people have had to travel overseas to get that done previously," she said.
The other two doctors taking up the training come from the internal medicine and radiology departments.
After they wrap up their trip, four more doctors from their hospital will join the training. Medical exchanges, including discussions of complicated cases, could still be carried out through telemedicine after the training, Xiao said.
Wu Dexi, a cardiologist at Xiao's hospital, spent three years in Dominica as a member of the Chinese medical team sent to the Caribbean country.
He twice voluntarily extended his stay there because, "I felt that many things had not been done as I had wished".
Wu helped establish Dominica's first cardiology department at the Dominica-China Friendship Hospital in 2021. The department has equipment such as ambulatory blood pressure monitors and temporary pacemakers.
He performed Dominica's first temporary cardiac pacemaker implantation on a 73-year-old woman soon after the department was launched.
Wu and radiologist Yang Peng, who is also from the First Affiliated Hospital SYSU, took Dominica's first contrast cardiac magnetic resonance imaging last month jointly with their local colleagues.
Wu also drafted Dominica's first thrombocytic procedure for acute ST-elevation myocardial infarction.
He treated about 5,000 outpatients in Dominica, which has a population of more than 70,000.
Improved treatment at the hospital also helped reduce some bills for patients, who might otherwise have needed to travel abroad for certain medical services, Wu said.
