预防性敷料实施注意事项有哪些？

一、在使用预防性敷料预防压力性损伤时，需要继续采取其他预防措施（如定期更换体位和支撑面）(专家意见）。

二、定期评估预防性敷料下的皮肤，每天至少一次，以评价预防性护理方案的有效性。许多敷料的特性有助于定期皮肤评估的实施（如透明性[1]，硅酮边缘[2-3]，非黏性边缘[4-5]（1级和3级证据）。

三、当选择预防性敷料时，考虑:

（一）使用敷料的潜在益处；

（二）合适的敷料尺寸和设计；

（三）敷料管理微环境的能力；

（四）易于粘贴和揭除；

（五）固定的能力；

（六）便于定期评估敷料下皮肤的能力；

（七）个人的喜好，舒适度和致敏性；

（八）和皮肤接触面的摩擦系数；

（九）成本效益和可获得性（专家意见）。

四、如果预防性敷料移位，松动或过度潮湿[2-3]，敷料或敷料下的皮肤污染，需移除敷料，根据产品说明书更换（1级和3级证据）。

五、对于有压力性损伤高风险的患者，在允许的情况下尽早使用预防性敷料（如在救护车或急诊室中即开始使用[2]（1级证据）。

01、优选进口硅凝胶，温和亲肤，不损伤皮肤，容易贴敷及去除；

02、反复粘贴，方便定期打开对皮肤状态进行评估检查；

03、保持皮肤适宜的温度与湿度、使压力再分布、减轻摩擦力，降低压力性损伤的发生率。
