【衡道丨病例】脑组织异位(Ectopic brain)如何诊断?

临床特点
1.年龄:先天性疾病,既往文献曾报道30例,除本例为成人(23Y)之外其余发病年龄均为婴儿或产检查出。无明显性别偏向(M: F=13:16)。
2.部位:头颈部-咽旁间隙(17/31,54.8%),颅内-硬膜外间隙(8/31),后腹膜(4/31),颅内外交通性(2/31)。
3.临床表现:呼吸窘迫(11/31,35.5%);无症状或仅存在外观不对称(10/31,32.3%);其他压迫性症状如喂养困难或打鼾等。

形态学诊断要点
1、大的错构瘤样团块,内部包含层次分明的脑膜-脑实质-脑室三层结构。 2、脑膜组织中硬脑膜、蛛网膜、软脑膜分化完整,清晰可辨。 3、脑实质内胶质、神经元成熟程度等同于患者年龄。 4、 脑室系统衬附室管膜,脉络丛有形成脑脊液的能力 (因此表现为囊实性肿块) 5、内部可见中枢神经系统内其他更复杂的高级及结构,如各类神经核团。

免疫组化:
主要表达中枢内各正常成分的免疫组化标记 如:神经毡(S100,GFAP,CD56);神经纤维(NF);神经元(syn,SOX10,NSE);脑膜皮细胞(EMA,SSTR2A)等。
分子检测:
暂无文献报道。
鉴别诊断:
1、畸胎瘤。畸胎瘤内常包含三个胚层的组织,且均发生于性腺或传统中线部位(后腹膜、骶尾部、颅内、纵膈);本病中的成分以神经外胚层组织为主,且绝大多数病例不发生于这些传统中线部位。
2、脑膜脑膨出。脑膜脑膨出、无脑畸形、脊柱裂等重大先天性疾病均起自于原始神经管闭合不全。原始神经管于胚胎4周闭合,若发生闭合不全则常发生于人体背侧。本病患者中枢神经系统背侧常发育完好,异位组织与神经管相连并沿体腔低压部位或各类先天间隙生长,与原始神经管闭合不全不符。
3、胶质-神经元异位。胶质-神经元异位最常见的形式是鼻胶质瘤,头面部最好发,但也可出现于全身其他部位,其内部以胶质成分为主,约10%病例可见少量神经元。但与本病不同的是它不含有脑膜、脑室的成分,更不会出现复杂的中枢内结构。
