背景
本研究旨在探讨右美托咪定(Dex)在体外循环中的应用。
方法
将60例行择期体外循环术的患者随机分为对照组(C组)和右美托咪定组(DEX组)。右美托咪定组麻醉诱导前于体外循环预充液中加入适量右美托咪定(1ug/kg),对照组给予生理盐水。测定不同时间点的平均动脉压(MAP)和心率(HR)。采用酶联免疫吸附测定试剂盒检测两组不同时间点(T0/T1/T2/T3/T4)的心肌肌钙蛋白I(CTnI)、丙二醛(MDA)、白介素6(IL-6)、肿瘤坏死因子-α(TNF-α)水平。
结果
两组患者术中、术后MAP和HR均明显低于术前(P<0.05)。右美托咪定组的MAP和HR显著低于C组(P<0.05)。两组患者在不同时点的cTnI/MDA/IL-6/TNF-α水平均显著高于T0时(P<0.05)。右美托咪定组T1/T2/T3/T4时血清cTnI、MDA、IL-6、TNF-α水平显著低于C组(P<0.05)。右美托咪定组心律失常发生率明显低于C组(P<0.05)。
结论
右美托咪定在体外循环预充液中具有稳定的作用(能有效控制血压,减轻心肌损伤,有利于术后快速恢复)。

Application of Dexmedetomidine
in Cardiopulmonary Bypass Prefilling
Objective:
The purpose of this study was to explore the application of dexmedetomidine (Dex) in cardiopulmonary bypass. Methods:A total of 60 patients undergoing elective cardiopulmonary bypass were divided into control (C) group and Dex group. In the Dex group, appropriate amount of Dex was added into the membrane lung prefilling solution before anesthesia induction, while those in control group were given normal saline. The levels of mean arterial pressure (MAP) and heart rate(HR) at different times were measured. The levels of cardiac troponin I (CTNI), malondialdehyde (MDA), interleukin 6 (IL-6), and tumor necrosis factor a (TNF-a) at different points (T0/T1/T2/T3/T4) in both groups were measured by enzyme-linked immunosorbent assay kits.
Results:
The intraoperative and postoperative levels of MAP and HR in the 2 groups were significantly lower than those preoperatively (P < .05). The levels of MAP and HR in the Dex group were significantly lower than those of the C group (P < .05). The levels of CTNI/MDA/IL-6/TNF-a at different points in both groups were significantly higher than those at T0 (P < .05). The serum levels of CTNI, MDA, IL-6, and TNF-a in the Dex group at T1/T2/T3/T4 were significantly lower than those in the C group (P < .05). The rate of arrhythmia in the Dex group was significantly lower than that in the C group (P < .05).
Conclusion: Dexmedetomidine has a stable effect in cardiopulmonary priming solution.
